// How To // UPDATED Applying for a nursing license in Illinois

UPDATE: In Sept. 2011, IDFPR made some changes to the application guidelines that affect internationally-educated nurses. Updated information is provided after each step, where necessary.

Here's a quick rundown of the things to do when applying for a nursing license in the state of Illinois if you're a foreign-educated nurse:

1. Apply for a Credentials Evaluation Service (CES) report from the Commission on Graduates of Foreign Nursing Schools (CGFNS). Request for a Course by Course evaluation or the Full Education report. Download the application form from the CGFNS Web site (http://www.cgfns.org).

*** You may apply for a CES with either CGFNS or with Education Records Evaluation Services.

2. If your school's medium of instruction is NOT English, apply and take the TOEFL iBT (www.ets.org/toefl). How do you know if you're school's medium of instruction is English? Aside from trying to recall if your teachers taught in English and made you read books in English, you can ask your registrar. The medium of instruction is required in the CES report, and this part will be filled in by your registrar or dean. (See instructions in the CES handbook in the application form.)

*** In the official IDFPR application guideline, it states that if your medium of instruction and textbooks are in English, the English language proficiency exam may be waived. However, I have not seen or heard of a case where the IDFPR has waived this requirement, so you may have to ask them personally as this will be reviewed on a case-to-case basis.
*** IDFPR now accepts IELTS exam scores, aside from TOEFL. In my opinion, you may want to go for IELTS as this test is recognized as well by other regulators outside the US, such as the UK, Australia, Canada and NZ, among others. TOEFL is only recognized mostly by US-based institutions.

3. Apply for state licensure by completing an application form (http://www.idfpr.com/dpr/apply/Rn.asp) and sending it to Continental Testing Services (CTS). Before sending your application form, request for a fingerprinting card from the Illinois Department of Financial and Professional Regulation.

To request the card, you can send an e-mail to Ms. West through her mail: FPR.PRFGROUP09(at)illinois.gov. Please ask kindly when you e-mail her.

*** It's been years since I last communicated with Ms. West and I am unsure if she still works for   IDFPR, so you may want to check this first. If you have new contact information, please send me a message so I may update this information.

When you receive your card, fill it up and go to NBI Carriedo. Buy a P20 ticket at the gate and then proceed to room 604. You will be given a piece of paper which you'll show to the fingerprinting person on the 5th floor. When you're done, send the fingerprinting card to Integrated Biometric Technology:

*** The NBI may have changed their fees and their step-by-step workflow. Again, if you have newer information, do share with us here.

Check IBT's Web site to find out how much they charge for the fingerprinting processing. Enclose a note to request IBT to send the receipt to CTS.

Each step requires you to pay certain fees. The list of fees is printed on the application packets. If you're in the Philippines, you can pay through bank draft. Go to a bank where you have an account and ask how you can get a bank draft. Before you go to your bank, make sure you know the exact name and address of the institution to which you'll be sending the payment and the amount, of course. Once you have the bank draft, send it together with your application forms to the respective organizations (CGFNS, CTS, etc.)

Common questions:
1. Do you need to take the CGFNS certificate exam?
No. You don't need the CGFNS exam. However, if you already have one, you can ask CGFNS to submit that to CTS in lieu of the CES.

2. Can you apply for the CES and the state licensure with CTS at the same time?
Yes. You can submit the forms at the same time, but your application won't be processed unless the initial steps are completed (CES then TOEFL then CTS application.)

3. Can you take the IELTS instead of TOEFL?
UPDATED Yes. The Illinois nursing board now accepts TOEFL AND IELTS scores. As mentioned earlier, if your CES report indicates that your medium of instruction and textbooks in school were in English, you may have the English test waived, HOWEVER, you still will need a passing English test score for the Visascreen application later on.

4. Fedex doesn't deliver to P.O. Box addresses. What address can I use to send my documents to CTS?

Hi Geof, I have several questions on how to fill out the application form for illinois:
1. Do I need to write anything like N/A to items which are not applicable to me?

2. It is stated in the reference sheet that the fee for CTS is $79 and NCSBN fee is $200, does this mean that I need to pay a total of $279 for the application?

3. In Part I, they asked for the Licensure Method, is the answer Examination CTS, NCSBN correct?

4. What does GED in Part III mean?

5. Are short term seminars like Hemodialysis in NKI, BLS, and IV therapy included in the Specialized training in Part III?

6. Do I need to declare that I took the local board last June and still waiting for the result?

7. Where can I find the possible answers to the boxes in the Part VII?

8. Is the Verification by Licensing Agency/Board form for PRC?

9. Do I still need to pay the additional $150 even if I will be taking the NCLEX here in the P.I.?

9. Lastly, any idea as to how much is the cost of fingerprinting in Biometrics?

I'll try to answer your questions:

1. you can either put NA or leave the item blank
2. when you send your application to CTS, include a bankdraft worth $79. You can get a bankdraft from your bank where you have an existing account.
2.a. you pay the $200 to pearson vue when you register for the NCLEX exam, but you can only do that when you already have your eligibility from CTS.
3. Just write "Examination"
4. I'm not sure what GED means, but you don't need to tick that question. Just tick Yes to the question if you have graduated from high school. the GED is an equivalent degree which we don't have here in the Philippines. Leave it blank.
5. Yes, you may include those training, but they usually don't accept them in the US. So, you can leave them blank if you want.
6. Yes, you have to declare that you took the local nursing exams. It's always better to be safe than sorry later on. By not declaring it, you are liable to perjury.
7. You can leave part VII blank.
8. The licenses verification board is the PRC. Get a form from the PRC (Yellow form for license verification--Window 1 & 2, I think), fill it out. IMPORTANT: You must have your PRC license CARD already when you ask for verification, otherwise PRC won't process your application. It's a system that SUCKS big-time, but it's how they do things.
9. Yes, you have to pay the additional $150 because you will be testing outside the US.

There's a page on the CES application form (part 3 or 4?) which you need to have your school registrar fill out.

There's also another page which you need to submit to PRC (if you took the local nursing board). This is the license verification page.

Hi! Is PRC license a requirement in requesting for CES report? Is it true that you can take NCLEX-RN even if u dont have a PRC license provided that you are a graduate of BSN? 'Coz my husband is a naturalized US Citizen, he graduated 10 years ago here in the Philippines. He took the Phil. Nursing Licensure Exam way back then but didn't pass. Is there a chance for him to be a licensed nurse in Illinois? Thanks....

Yes, a Philippine nursing license is NOT required if you wish to register as a nurse in Illinios.

HOWEVER, in your husband's case, I think there are special requirements if you haven't practiced nursing for 5 or 10 years. I'm not sure exactly what the reqt's are, but please contact the Illinios Dep't of Financial and Professional Regulation directly to ask about your husband's case. Once you get your eligibility you have one year to register with Pearson Vue, but after you register and get your ATT, you only have 90 days (3 months) to schedule an exam. Usually 90 days is enough, especially with the Manila testing center now scheduling exams.

HEllo! I am planning to apply in Il. I just want to ask about the CES... Do i still need to apply for the CES eventhough i passed my CGFNS exam already?
2nd question that bothers me is that, where can I get the SSS#?
Lastly, I'm living in the province, and i'm not familiar with NBI Carriedo.Where can I find NBI Carriedo? Is in in Manila? Can I go to the NBI here in our province instead if NBI Carriedo?

You don't have to get the CES if you already have the CGFNS certificate. Just ask CGFNS to forward your certification to CTS. I'm not sure how much CGFNS charges for this service but it shouldn't be expensive.

You cannot get the SS Number unless you already have an approved immigrant application. I would focus on getting all the reqt's to be able to work in the US first because once you already have an approved visa, you will be issued an SSN.

I would assume that provincial NBI offices also process the fingerprint cards. Inquire at your local NBI office first so that you won't have to travel to Manila.

your article was indeed enlightening, however, i still got few questions left;

1. i laready applied at CTS but unfortunately it lacks fingerprint. Last June 30, i was asked to submit them. I send a copy to my father in Illinois for him to obtain a livescan in any of the vendors indicated in the application forms. Approximately how long will it still take for my eligibility to be comeout after it has been submitted.
2. I got my CES report last week of July and it was indicated that my Language of instruction was english and english textbooks. As you said I no longer have to take the TOEFL. I took the IELTS instead and got a band score of 7.5, can I use that for my visa screen?

Lastly, can I already apply for visa screen using my IELTS and CES report on hand?

Your assistance will be greatly appreciated.

Are you in the Phils.? You'll need to request the fingerprint cards with IDFPR. The vendors don't have those cards. The Livescan is for those who can go to the center personally. Look for the e-mail address of the IDFPR contact person in one of my posts and request for a fingerprint card.

Yes, you can use the TOEFL iBT for the Visascreen application. However, you can't use the CES to apply for a Visascreen. You'll need a passing NCLEX score and then your English test for this.

yep, I am still in the Phils. I got my fingerprint card from the US Embassy. Is it alright to use them?

And provided I have already submitted them, would you know, approximately when can my elibility come out? or how long will it still take for me to wait for it come out?

What I have is IELTS and not TOEFL as an english exam.

Is it alright to use them with an NCLEX exam to apply for Visascreen?

Yes, you can use the fingerprint cards provided by the US embassy. When you send the cards, include a bankdraft to pay for the processing fee. Also, include a note, asking the provider to forward a receipt to CTS.

Once that's done, you may get your eligibility in two weeks or more (if all other reqt's have been met already).

If you want to register with Pearson Vue, you can do so even if you haven't received your eligibility. Pearson Vue will only release your ATT once they get the go-ahead from the Illinois board, but you are allowed to register concurrently while your eligibility is being processed.

It should arrive anytime soon. Sometimes it takes two hours or more than two weeks. If four weeks have passed and you still don't have your ATT, you can call CTS.

Occassionally check your spam box just to check if it's redirected there.

If your medium of instruction in college was English, you can be exempted from the TOEFL score req't.

With CGFNS, theer's a code for the Illinois board that's indicated in the application packet. E-mail yur chosen service provider (ERES) for the correct code, if they have one. Otherwise you can just put in IDFPR or CTS

I hope you know that you have been very helpful for us. anyway, I still have a few inquiries.
-If Illinois allows applicants to go on with their registration as an Illinois nurse, eventhough they are not licensed in the Philippines, can they apply for a CES even there is no license?
-in the IFDPR application, there is what they call a CT-NUR. It should be returned to the applicant, right? How come in the form it is written "Do not return to applicant"?
-Do I need to accomplish the ED-NUR? From my understanding, if you're an international applicant and has to submit a CES, you do not need to submit an ED-NUR, Yes?

Oh, and I just want to share, Provincial NBI's do not process Nurse fingerprints, only the Carriedo branch does that. At least in Bulacan's case, that is.

Thank for the tip about fingerprinting. To answer your question, if you don't have a local license there's no need to submit the CT-NUR which is the license verification. Just e-mail both CGFNS and CTS that you don't have a local license. Occassionally, they'll ask you again about your local license, just answer them back. And if they ask again, just tell them again. In some cases their records are not updated fast enough.

bert, you don't have to wait to be issued a CGFNS ID. complete all your documents in the soonest possible time and submit them as soon as they're available.

unfortunately, you have to wait until you have your plastic PRC card before you can apply for license verification with the PRC. once you have it, photocopy it and fill out a form at window 1 or 2 at the PRC office. ask them about Fedex delivery. A Fedex rep will pick up your documents from the PRC directly.

If you already the visascreen without taking NCLEX, that means you have the CGFNS Certificate, right? Then you can use the CGFNS Certificate in lieu of the CES. Ask CGFNS to forward your Certificate to CTS

Hi. I'm glad you find my blog useful. To answer your questions:
1. Your tourist visa may not help fast-track your registration application to the US. You may use it to visit the US and possibly explore opportunities, but my advice is for you to "never be out of status" there, meaning, don't overstay if you don't want to compromise your future as a nurse.

2.You can send your application for the CES and CTS at the same time, but CTS will still wait for the CES report before it issues your eligibility.

3. The ED-NUR is to be filled out by your school's registrar and then sent to CGFNS directly by the school. ***Please go to nclex-rn.net/nclex forum to ask about a faster way to do this, as I can't mention it here.

4. You'll need your PRC license card to be able to request verification of license from the PRC. Just photocopy your license and fill out a form in Window 1 or 2 at the PRC. Then ask about Fedex delivery if you want to use one-day delivery service.

5. Just use ordinary letter-sized bond paper. Doesn't matter if the print is in color or not.

6. Our medium of instruction in the Philippines is English so you won't need the TOEFL before being issued an eligibility. Ask your registrar to be sure because this will be noted in the ED-NUR and reflected in your CES report.

im a lil bit confuse regarding ED-NUR, on the instruction in the idfpr it is stated that ED-NUR is for those who educated in US but in continentaltesting.net it said that "All candidates must provide proof of education using the Illinois ED-NUR Form" which will i follow? by the way i finish my education in Philippines.

another thing is according to continentaltesting.net "This exam requires a double application. The candidates need to file an Illinois Application with CTS and also register with the NCLEX Provider Pearson Vue." so does it means that i have to comply 3 applications? 1 from idfpr.com, another one from continental and pearsonvue??? or just two? continental and pearson only and no need to fill up the idfpr application forms???

Unfortunately, you need to wait until you have your PRC license card with you. PRC won't accept request for validation without it.

You should send the documents that you already have while waiting for other requirements. Although your final report will only be issued until you complete all of the reqts, it's best that you send available documents right away and pay for the service so that they can already have your application on file.

You have a year to complete everything and that's enough time for anyone. It doesn't usually take that long anyways.

1. You can send your Request for Validation at a later time when it becomes available, so I suggest you send your CES application and payment right away and then enclose a note that your Request for Validation will follow.

CTS will not give your eligibility without the CES report, so you can't take the NCLEX yet. When you get your license validation, the eligibility is usually issued in less than a month or as early as two weeks. Often there is just some delay in the postal mail.

2. Check your high school diploma. Some have KATIBAYAN written, others KATUNAYAN.

3. If you're not a second-courser or if you didn't attend any other school except your nursing school, then you don't have to write anything. For all questions that are inapplicable to you, just leave it blank or write NA

About the NCLEX Application

1. The zip code of your postal address. The 9-digit code is for US territories. We use a 4-digit code. So write your town's zip code.

2. Just leave it blank if you didn't have a name change.

3. For questions that are inapplicable tou you, just leave them blank.

4. Usually the CES report will suffice and you don't need to submit the license validation to CTS. Your CES report will reflect your license already. You can still send it if you want though. The same process with the PRC applies for license validation.

5. And how should I submit the forms to make the most out of the available time? Submit your CES Report application first with your payment though credit card or bank draft. Then submit your application form to CTS. They will both send you mail informing you of your deficiencies, which should be your license validation. Then once this is available, ask PRC to send it right away. Use Fedex if your budget allows.

While waiting, I suggest you review for your English test. IELTS or TOEFL iBT. Once that's done, go for hospital training. Then once you get your eligibility, start formal review for the NCLEX. However, you should already start studying on your own at this time.

The CES report is issued by CGFNS and then forwarded to CTS. The Illinois board actually accredits two other agencies to issue the CES report, but I suggest you go with CGFNS because other states will only recognize CGFNS just in case you need your CES report for another state.

About the ED-NUR

If you already have the CES Report, there's no need to submit the ED-NUR. E-mail CTS directly if you want to be extra sure. But I didn't submit the ED-NUR anymore.
Your education background is alredy included in your CES report.

Licensing with Illinois includes several steps:
1. apply for CES report with CGFNS
2. apply for licensure with CTS
3. apply for NCLEX exam with Pearson Vue (once CTS gives your eligibility)
4. apply for license registration with CTS (this is after you have already passed NCLEX)

1. Is the secondary eduction equivalent to hs or college?

2.Is it still advisable to take the ielts or toefl even if it is not required for eligibility? Or it is just for future use for the visa screening?

3. does illinois require work experience or training when they hire new nurses?

4. How long will it take to receive the eligibility and the ATT? will there be ample time to work here in P.I. to have work experience?

5. how much will I put in the Part I letter A number 4 of the apllication for licensure? the fee?

6. What about part 4 and part 5 of the application for licensure? What is the best thing to do? Would it be best if we wait for our license or pass the application just to make the process moving?

hi! first of all i want u to know hw happy i am that ive i found your site, the procedures that uve provided really clarified all my questions regarding how to take an nclex at illinois, so i thank you, my situation is this, im a newly licensed nurse(june 2007) and since we have a large volume of newly nurses(around 35thousand)the PRC has a SLOW rate of issuing our License number/card, b'coz of this, im having second thoughts on my application on CTS or wid CES, so my questions are; can i skip the verification part of my license and just passed my application??will it not deter my application?? or being a Phil Board passer is one of d requirement for the said application??thanx!!!!

1. secondary ed is equivalent to our high school education
2. although you won't be required to take the toefl, you will still need a passing english test score for your visascreen. you can take either IELTS or TOEFL iBT. I suggest you to take the english test while waiting for your Illinois eligibility since you have to wait for your PRC license to arrive.
3. you need work experience to get a license in Illinois. when you apply for work, however, some recruiters might require that you have work experience. so for licensing, no you don't need a license. for work, yes and no, depending on your desired employer.
4. the longest wait is for your PRC license to be available, once you have that, it'll take usually about less than a month before your eligibility is issued.
Without your PRC license, you can't work here in the Phils. You can however enrol in some training programs. Look around for programs that won't require your PRC card.

5. sorry I don't have an Illinois app packet with me right now. but if you want to know about the application fee, I think it's $79 to CTS and $54 for the fingerprint processing with IBT. Go to CGFNS.org for the CES report fee.

1. Can i submit now my CES to cgfns? Do i have to enclose the receipt of the bank draft? Do i also enclose my highschool and college diploma (I am a nursing second courses btw)

2. The request for academic records for CES are now sealed and ready for mailing along with the other requirements like the TOR, diplome etc.. arnold, if you request the fingerprint cards now, it usually takes at least two weeks to arrive, sometimes earlier, sometimes longer.

there is no additional fee when requesting the fingerprint card, but you have to get the services of IBT (Integrated Biometrics) or other accredited providers who will scan your cards. The last time I looked IBT charges $54 for their service.

2. If you plan to go to a formal review class for your NCLEX, I would suggest that you schedule it as close to your testing date as possible. We tend to forget the things we study if there's a long gap.

Get the English test out of the way first if you expect to wait a long time for your eligibility. Enrol in some training seminars as well. Some people have trouble studying while working at the same time, so consider this option carefully. Hospital applications take several months anyway so by the time you get hired, you might have already taken your NCLEX.
3. I don't have experience with IELTS so I can't give you my personal feedback. But other people say IELTS is easier. I really can't say. Why don't you look at sample exams available on the Internet and see at which test you think you'll be able to perform the best.

Some of your questions have already been answered in my earlier posts, but to guide you along, here's my quick reply.

You can already start sending your application forms, including the education form, to CGFNS. Enclose the bank draft and copies of your diplomas.

If you know someone else who's also applying for Illinois, you can send your documents together in one Fedex envelope. Just have separate brown envelopes. This way you can share the cost of shipping.

But the formal review of your documents won't start unless you have completed everything, including the license validation form. so, the long wait is really only for your license validation form.

Wouldnt it be unfair for those who took the locAl board because the process of the issuance of license will take a couple of months. What about the BSN grads who didnt take the local board.. Were they aproved by the cgfns for cesr even without the license vreification?

you're right, a local license is not required for registration in Illinois, BUT if you have taken it you have to declare to the Illinois board that you took the exams, whether you passed it or not. This is the "legal" thing to do. You don't want something like this bite you later when you have already processed everything. This is something you have to think about seriously if you're thinking of shortcuts.

Those who didn't take the boards actually finish their applications MUCH faster. They get their eligibility in about 2-3 months. But if anyone is thinking about hiding their license status, better think about possible consequences later on. There might be none or you could end up in a very deep hole that's hard to crawl out of. You have to weigh your choices everytime.

My friendly advice is always go for the right thing. Goodluck to you.

That's actually a feasible option. Pwede...pero.. it's a very expensive way to get a license if your end goal is to work in Illinois. If you endorse from another state to Illinois, you'll still need to secure a CES report, so you'll pay for your application in the first state and then pay for your licensure application in IL.

Actually the entire processing time to get a license in IL by going through endorsement instead of just waiting for your PRC license is almost the same. The only advantage I see is you'll probably take your NCLEX faster if your chosen state doesn't require a lot of documents.

Hello Geoff, my case is the same with Nico, Im a June 07 passer and as you said we still need to wait for license verification from PRC. I already submitted a CES application and declared that I took the Phil NLE and passed the examination but due to bulk number of applicants my sked for Registration is on November and license will most likely be released 2 to 3 months from registration date. Have not received any response from them yet because that was just 2 days ago. Now my concern is, do I already need to submit my CT NUR application or just wait for CGFNS response first before I do the next move. Thanks.

you'll need to declare your first course. if i remember it correctly you should put it down in the non-professional education part of the application form.
i'm not very sure about license validation for non-nursing licenses. my guess is they won't require it anymore. however, please please email cts just to be sure. you can fidn the contact info in the resources page of my blog.

The Illinois State Board requires TOEFL for non-English native speakers. Filipinos are exempt from this requirement, however because our medium of instruction is in English. You must make sure that your CES report reflects this. In the education verification form of the CES application, you'll find the part that asks about your school's medium of instruction. your school registrar fills out this form.

So, you can take either TOEFL iBT or IELTS. As long as you get the required scores, you won't have any problems.

The CT NUR form is the license validation right? You submit this to the PRC and then PRC will send it to CGFNS. CGFNS usually responds to your mail but it can take from 3 or more weeks; there's no definite response time but they do reply.

Like I said, you'll need the PRC license card to complete the license validation form

we took the toefl ibt because we were considering going to grad school in the US. but if you ask me whether ibt is harder than ielts, i really wouldn't know based on first-hand experience because i haven't taken the ielts yet. from what i gather from other people, more people are comfortable taking the ielts. i think if you're not very confident about your english skills, you should follow the advice of others and take the ielts.

for most states, well, at least 23 states, you don't need the CP anymore. but if you really want to take it, I know that the CP already includes credentials evaluation and you won't need a separate CES application. Just pay for the CP and that's it. For states that require only the CES, you can use the CP instead.

Check the application packet, what info you need to put in the bank draft. For example, if the check should be made out for which agency. Then go to a bank where you have an account. If you don't have one, you can ask your parents to get one for you. Just ask the bank rep how to get a bank draft and they will walk you through the process. Bank payments are usually debited from the account so make sure your account has enough funds.

1. You can put CTS or Continental Testing Service as your receipient.
2.Check your diplomas, if it says Katunayan or Katibayan or if its in English, then write that down.
3. You're applying basically for licensing or you can put in employment in the purpose of your report
4. Part IV I think is the license verification. You need to wait til your local license is released (if you took the boards) and then have the PRC handle that form. Read some of the comments above for more info on this.
5. Child support does not apply to us, so just put N/A or No.

For endorsement, I think you still need to get a CES report, if you still don't have one or the CGFNS certificate.

Try to email IDFPR directly. They usually answer in a day or two. You can find the link in the Resources page.

The process is fairly fast. The longest wait is for the License Verification. The PRC doesn't issue verification unless you already have your license card and that can take a long time. But once you have all the requirements for the CES, it can be issued in less than a month. Your eligibility will also be issued in a few weeks.

Because there is a long wait for the license verification, what we did was to submit all the other documents that were needed for the CES and while waiting, we took the English test first. This way, no time is wasted, so to speak.

Hayy.. medyo nalilito talaga ako kung paano mag apply ng nclex sa IL. I already my fingerprint card.

Another question, ilang days bago mag expire ang ATT? I just want to have an idea at least mai budget ko yung review days ko. Is it 1 year ba? I read from somewhere it varies from state to state, some its 1 year others 60 days and 90 days.

IL license application is pretty straightforward. there are more than one agency that you have to apply for that's why it gets a bit complicated. but whenever you encounter a problem, just post a message here and I'll try my best to answer them.

The ELIGIBILITY expires in 1 (one) year, you have to register with Pearson Vue within that time. But once, you have already registered and paid the $200 testing fee to Pearson Vue, they will issue you the ATT, which is valid up to 90 DAYS. You have to schedule an exam date before your 90 days is up.

When you claim your license card in PRC, bring the License Verification form from your CES application. Fill up the appropriate data. Photocopy your license and go to Window 1 or 2 (I think Windows 3 or 4 also) and show them the form. They will give you a Yellow Form. Fill that out and I think you have to pay for the stamp (I forgot what it's called.) Ask about Fedex shipping and they will either schedule pickup and delivery. Bring about $1,500 for Fedex services. If you have a friend who's also applying for IL, you can ask that your docs be sent together. This way, you'll be able to share the cost for mail services.

Print out all the application form, including the application form that you've downloaded from CGFNS and IDFPR.

If you apply for express service, you can get it within the day. You can choose between the regular courier service or Fedex. Ask about the Fedex service. Usually, your document will be scheduled for pickup and you'll pay the Fedex rep directly.

#5 if what i downloaded is correct, can i just submit "CT-NUR" and "Request for Validation of Registration/License for CES" on the same day, same thing for "ED-NUR" and "Request for Academic Records for CES"?

#6 am i going to do the right thing then or am i messing the process for application lmao? `coz i'm expecting the fingerprint cards to be delivered next week, after i received the fp cards, i'll process my nclex application.

For questions 1 to 4, you've done everything right.

For #5, just accomplish the Education review and License Validation for the CES application.

Technically, you can send the Education and License forms the same time. But you'll need to have your PRC Nurse ID ready before you can do license validation. Check out the posts above for more info on License Validation.

you can opt to accomplish just the license and education verification for the CES and disregard the same forms from the IDFPR application.

you may choose to send the license validation to IDFPR as well, but it's not really necessary because the same information will be reflected on your final CES report.

Thanks a lot :D I understand already that completing all forms in the IDFR is not necessary, so I may skip some forms because of CES verifcation will already suffice. So, all in all, I made a planner already. You may want to check if i'm missing something :D

#1 Once i get my fingerprint card, i'll go right to nbi and make a bank draft and send it to IBT with a note to send it to CTS afterwards, btw, i sent an email inquring about how much would it cost, the lady said its down to $49 something

#2 If my time permits, I'll go to PRC, photocopy my license, fill out a form in window 1 or 2 and ask for fedex delivery w/c would cost P1,400 something

#3 Again, if my time permits me, I'll go the school registrar and request for the academic records.

(I am thinking of processing this 1 day, if it's possible)

But I still have some questions:
#1 Is the school, the one responsible for sending the CES academic records to CGFNS or should I send it instead?

#2 What if it will take some days/weeks before the school can process the CES academic records, then that would delay the process? If that is the case that the CES academic records would be delayed, can I still send the IDFR Application for Licensure Exam w/o the CES academic records from the school, provided that I already sent CES licensure from PRC? Or should I just wait for all the forms to completed and send them all at the same time?

#3 Are there anything else that I need to include (photocopy or original forms etc.) when:
a. I send the CES Registration to CGFNS?
b. I send the IDFR application?

#4 Is it required to use special paper or ink to print the forms? Must it be colored (for CES?

the school should send your docs, but if their policy is to let students do it, ask them to seal the docs in official school envelopes, so you can send it thru the nearest courier service. usually, from experience it takes the school a couple of days to months to prepare the TOR and fill out parts of the CES form, so I suggest to use charm and wit to convince to work faster. Har har har

Kidding aside, you can send your docs as they become available. You'll be sending the TOR, License Validation and application separately, and also the fingerprint card, so send whatever document you have ready.

No need to print in color or use special ink. Blank ink will do just fine.

It's better to finish everything before you leave. This way your eligibility will be ready when you reach the US.

There have been cases where someone on a tourist visa was sent home, perhaps there was a reason for the immigration officer to suspect that the person is intending to stay in the US for work and not just to visit. An eligibility to test, or even a passing NCLEX score, will not show up on the immigration officer's computer, but other things, for example declaring that you are a nurse or other physical evidences in your possession at the time of the interview, may be used however.

I would assume Fedex will any type of document. They normally don't go through every page of your parcel so it doesn't matter.

Place your reveiew materials inside an envelope if you wish.

the fingerprint card should be sent to IBT. please check with IBT directly how much their rate is now, because it might have changed.

I'll ask one of the past readers here to answer your question also about IBT's rate.

The authorization is needed when you can't personally manage your own application. for example, i you have a lawyer or perhaps a relative whom you want to receive your documents or call CTS in your behalf.

Just leave that blank. :)

Your school records should be sent by your school. however, some schools don't have their own mailing service, so you must ask the registrar to put your documents in a sealed envelope and you should send it at the nearest postal office or courier service. asking a relative to send it from the US will only create suspicions that your documents may not be authentic.

with regard to your relative sending documents in your behalf: unless your relative is enclosing a check to pay for the services, I highly suggest that you skip it entirely and send your application directly to the appropriate agencies. sending it to someone else only creates delays and opens up other risks such lost mail or crumpled forms, among others.

On second thought, even if a relative who's in the US is paying for your application, things can move faster if you send your documents directly to the appropriate agencies. Just ask your relative to call the agency and ask how or where to send payment for someone else's application.

Payments to CTS can be usually done using a bank draft or international money order, if an applicant is in the Philippines. Otherwise, a check, money order, or cash paid directly at the agency's office can be used if the person is in the US.

The TOR should be sent together with the education record forms.

Im about to mail my application to CTS. I'd like to ask what else shall I include there?

a. 4 page application form
b. Ed-nur
c. TOR (does it have to be original?)
d. Certified true copy of diploma
e. Fp-nur
f. bankdraft

Am i forgetting something else? Do i still need to include board certificate from PRC? I've submitted everything except for the application for CTS. Can you help me please? I want to submit everything in just one mailing, since mailing fees are very costly.

**ibt has already sent out confirmation receipt to CTS, but i havent mailed my application to CTS. Is that ok? Mas nauna pa yung fingerprint card ehehe

I've always held the opinion that it is simply better to apply directly in the US state where you want to work in. This is my own opinion and I won't tell you to follow it.

Weigh the pros and cons and then make a decision. Whatever you decide on, we'll try to help you get things organized.

i think the ed-nur, if you still want to send it, goes with the TOR and these documents should be sent by your school. Or if your school doesn't have its own mailing service, have your registrar place them in a sealed envelope with official school marks.

but you're right, you don't need to send the ed-nur form since your CES report will reflect the information CTS needs.

just want to ask when will my illinois eligibility be issued. it's been almost two months (7 weeks to be exact)now and still i dont have my eligibility. they've received my application and payment last nov. 1 ( based on fedex's on-line tracking)... i've received my ces report last nov. 8 so i assume cts also have a copy of it already. i also have a confirmation of fingerprint processing from identix. does cts correspond through e-mail? i've always been checking my e-mail but to no avail. do they send you a letter stating they have received your application requirements? i'm totally clueless about the status of my application. i'm done with my review so i really want to take the nclex soon... btw, you've been a great help. thank you very much!

it usually takes CTS sooner than two months to process your eligibility. i'm sure there are updates to your application. there could just be some delays in the mail service. I would suggest calling them or e-mail them and ask the status of your application.

are you ready to take the nclex? since you've got most of the reqt's done, I would also suggest that you register with Pearson Vue already so that you can get the next step in the process started. Remember though, that once Pearson issues an ATT, you have three (3) months to schedule an exam.

i agree, that part gets a bit confusing. the application forms, the education record, the license record, and the fingerprint card should be sent separately. if your school can't mail your records, have the documents sealed and mail it yourself. the license record, however, must be sent through the PRC. once you get your license verification form, you have the option of either sending it through regular mail or Fedex right inside the PRC compound. A Fedex rep comes to the PRC everyday but ask the PRC staff for the next pickup schedule. You pay directly to the Fedex rep.

Geoff help, completed all the requirements for CES but when I checked the status of my application thru CGFNS website, read there that they sent me a letter via standard ground mail that the date of program completion in my transcript is inconsistent with what was written in my application. If I remember it right what I wrote in my application was the date of my graduation which is April 20, 2007. In my transcript date of completion was March 31, 2007, S.O. dated April 20, 2007. What shall I do? Will this disqualify me from taking the NCLEX in Illinois?, please help me, I'm having sleepless nights. Thanks

This is a common error which usually delays the release of your CES. No need to fret, this won't disqualify you. You will need to submit another set of application forms. Check which form (CES application form vs. Education Record form) has the wrong data and re-submit those with the correct information.

I think its the CES Academic Records Validation, I checked the photocopy Im holding right now, on the upper portion I wrote there date of attendance is from June 2004 to April 2007 and on the lower portion filled out by our reg, date of attendance is from June 2004 to March 31, 2007. So do I get you to mean that I need to fill out another CES Academic Validation form and asked the reg to fill it out again and send it to CGFNS? What about the transcript? Do I need to send another copy? Thanks again, you're really heaven sent hehehe.

Date of attendance and date of graduation are two different things. You might have been confused when you were asked about the two dates.

I'm not sure if you still need to send another form or if CGFNS would just coordinate with your school. Better to ask them directly by sending them email, which would be the best course of action at this time.

If you're applying for a license in IL, you can get the Healthcare profession CES report which is cheaper at $328. But if you want the course-by-course report, then that's $378. Each of these amounts already include the $50 processing fee.

The confusion comes from a recent fee change implemented by CGFNS. Previously quoted fees did not include the additional $50 processing fee. Now they've added the additional fee in their literature.

I just receive a confirmation letter from CTS and its also indicated that they havent received any docs from CGFNS yet. My CGFNS status is still "ready for review". Will there be any problem with that? I mean, ive submitted everything to CGFNS and CTS. Or shall I contact CTS about it?

focus on studying for the nclex instead, so that when your eligibility arrives, you'll be prepared.
