Here's a quick rundown of the things to do when applying for a nursing license in the state of Illinois if you're a foreign-educated nurse:

2. when you send your application to CTS, include a bankdraft worth $79. You can get a bankdraft from your bank where you have an existing account.
2.a. you pay the $200 to pearson vue when you register for the NCLEX exam, but you can only do that when you already have your eligibility from CTS.
3. Just write "Examination"
4. I'm not sure what GED means, but you don't need to tick that question. Just tick Yes to the question if you have graduated from high school. the GED is an equivalent degree which we don't have here in the Philippines. Leave it blank.
5. Yes, you may include those training, but they usually don't accept them in the US. So, you can leave them blank if you want.
6. Yes, you have to declare that you took the local nursing exams. It's always better to be safe than sorry later on. By not declaring it, you are liable to perjury.
7. You can leave part VII blank.
8. The licenses verification board is the PRC. Get a form from the PRC (Yellow form for license verification--Window 1 & 2, I think), fill it out. IMPORTANT: You must have your PRC license CARD already when you ask for verification, otherwise PRC won't process your application. It's a system that SUCKS big-time, but it's how they do things.
9. Yes, you have to pay the additional $150 because you will be testing outside the US.
There's also another page which you need to submit to PRC (if you took the local nursing board). This is the license verification page.
HOWEVER, in your husband's case, I think there are special requirements if you haven't practiced nursing for 5 or 10 years. I'm not sure exactly what the reqt's are, but please contact the Illinios Dep't of Financial and Professional Regulation directly to ask about your husband's case. You cannot get the SS Number unless you already have an approved immigrant application. I would focus on getting all the reqt's to be able to work in the US first because once you already have an approved visa, you will be issued an SSN.
I would assume that provincial NBI offices also process the fingerprint cards. Inquire at your local NBI office first so that you won't have to travel to Manila.

your article was indeed enlightening, however, i still got few questions left;
1. i laready applied at CTS but unfortunately it lacks fingerprint. Last June 30, i was asked to submit them. I send a copy to my father in Illinois for him to obtain a livescan in any of the vendors indicated in the application forms. Approximately how long will it still take for my eligibility to be comeout after it has been submitted.
2. I got my CES report last week of July and it was indicated that my Language of instruction was english and english textbooks. As you said I no longer have to take the TOEFL. I took the IELTS instead and got a band score of 7.5, can I use that for my visa screen?
Lastly, can I already apply for visa screen using my IELTS and CES report on hand?

Are you in the Phils.? You'll need to request the fingerprint cards with IDFPR. The vendors don't have those cards. The Livescan is for those who can go to the center personally. Look for the e-mail address of the IDFPR contact person in one of my posts and request for a fingerprint card.
Yes, you can use the TOEFL iBT for the Visascreen application. However, you can't use the CES to apply for a Visascreen. You'll need a passing NCLEX score and then your English test for this.

Once that's done, you may get your eligibility in two weeks or more (if all other reqt's have been met already).
If you want to register with Pearson Vue, you can do so even if you haven't received your eligibility. Pearson Vue will only release your ATT once they get the go-ahead from the Illinois board, but you are allowed to register concurrently while your eligibility is being processed.
i already applied with Pearson, I was hoping to take the nclex soon.

With CGFNS, theer's a code for the Illinois board that's indicated in the application packet. E-mail yur chosen service provider (ERES) for the correct code, if they have one. Otherwise you can just put in IDFPR or CTS

unfortunately, you have to wait until you have your plastic PRC card before you can apply for license verification with the PRC. once you have it, photocopy it and fill out a form at window 1 or 2 at the PRC office. ask them about Fedex delivery. A Fedex rep will pick up your documents from the PRC directly.

1. Your tourist visa may not help fast-track your registration application to the US. You may use it to visit the US and possibly explore opportunities, but my advice is for you to "never be out of status" there, meaning, don't overstay if you don't want to compromise your future as a nurse.
2.You can send your application for the CES and CTS at the same time, but CTS will still wait for the CES report before it issues your eligibility.
3. The ED-NUR is to be filled out by your school's registrar and then sent to CGFNS directly by the school. ***Please go to nclex-rn.net/nclex forum to ask about a faster way to do this, as I can't mention it here.
4. You'll need your PRC license card to be able to request verification of license from the PRC. Just photocopy your license and fill out a form in Window 1 or 2 at the PRC. Then ask about Fedex delivery if you want to use one-day delivery service.
5. Just use ordinary letter-sized bond paper. Doesn't matter if the print is in color or not.
6. Our medium of instruction in the Philippines is English so you won't need the TOEFL before being issued an eligibility. Ask your registrar to be sure because this will be noted in the ED-NUR and reflected in your CES report.

You have a year to complete everything and that's enough time for anyone. It doesn't usually take that long anyways.
You might have noticed, this comments page has been a bit in a disarray. So that I can properly answer your questions and not get my answers mixed up in someone's else's questions, please leave your name or Web alias. This way I can properly label my answers. Thank you.
1. You can send your Request for Validation at a later time when it becomes available, so I suggest you send your CES application and payment right away and then enclose a note that your Request for Validation will follow.
CTS will not give your eligibility without the CES report, so you can't take the NCLEX yet. When you get your license validation, the eligibility is usually issued in less than a month or as early as two weeks. Often there is just some delay in the postal mail.
2. Check your high school diploma. Some have KATIBAYAN written, others KATUNAYAN.
3. If you're not a second-courser or if you didn't attend any other school except your nursing school, then you don't have to write anything. For all questions that are inapplicable to you, just leave it blank or write NA
About the NCLEX Application
1. The zip code of your postal address. The 9-digit code is for US territories. We use a 4-digit code. So write your town's zip code.
2. Just leave it blank if you didn't have a name change.
3. For questions that are inapplicable tou you, just leave them blank.
4. Usually the CES report will suffice and you don't need to submit the license validation to CTS. Your CES report will reflect your license already. You can still send it if you want though. The same process with the PRC applies for license validation.
5. And how should I submit the forms to make the most out of the available time? Submit your CES Report application first with your payment though credit card or bank draft. Then submit your application form to CTS. They will both send you mail informing you of your deficiencies, which should be your license validation. Then once this is available, ask PRC to send it right away. Use Fedex if your budget allows.
While waiting, I suggest you review for your English test. IELTS or TOEFL iBT. Once that's done, go for hospital training. Then once you get your eligibility, start formal review for the NCLEX. However, you should already start studying on your own at this time.
If you already have the CES Report, there's no need to submit the ED-NUR. E-mail CTS directly if you want to be extra sure. But I didn't submit the ED-NUR anymore.
Your education background is alredy included in your CES report.
Licensing with Illinois includes several steps:
1. apply for CES report with CGFNS
2. apply for licensure with CTS
3. apply for NCLEX exam with Pearson Vue (once CTS gives your eligibility)

2. although you won't be required to take the toefl, you will still need a passing english test score for your visascreen. you can take either IELTS or TOEFL iBT. I suggest you to take the english test while waiting for your Illinois eligibility since you have to wait for your PRC license to arrive.
3. you need work experience to get a license in Illinois. when you apply for work, however, some recruiters might require that you have work experience. so for licensing, no you don't need a license. for work, yes and no, depending on your desired employer.
4. the longest wait is for your PRC license to be available, once you have that, it'll take usually about less than a month before your eligibility is issued.
Without your PRC license, you can't work here in the Phils. You can however enrol in some training programs. Look around for programs that won't require your PRC card.
5. sorry I don't have an Illinois app packet with me right now. but if you want to know about the application fee, I think it's $79 to CTS and $54 for the fingerprint processing with IBT. Go to CGFNS.org for the CES report fee.
6. I suggest you submit your application already, and submit the license verification later on when it's available. The education report can be easily completed by going to your school registrar.

i understand your predicament, but you have to declare that you took the local exams and passed. so you have to verifiy your license. this is really the right way to do it and the safest way if you wish to avoid potential complications in the future.
but I also suggest you explore other options and study them very carefully. the final decision is really up to you, but before you decide what to do, study it carefully.

there is no additional fee when requesting the fingerprint card, but you have to get the services of IBT (Integrated Biometrics) or other accredited providers who will scan your cards. The last time I looked IBT charges $54 for their service.
1. If you've already completed all the documents, including the license validation form, then expect your eligibility in 3 to 4 weeks in the mail. They process fairly fast and there is just some delay in the mail.
2. If you plan to go to a formal review class for your NCLEX, I would suggest that you schedule it as close to your testing date as possible. We tend to forget the things we study if there's a long gap.
Get the English test out of the way first if you expect to wait a long time for your eligibility. Enrol in some training seminars as well. Some people have trouble studying while working at the same time, so consider this option carefully. Hospital applications take several months anyway so by the time you get hired, you might have already taken your NCLEX.
3. I don't have experience with IELTS so I can't give you my personal feedback. But other people say IELTS is easier. I really can't say. Why don't you look at sample exams available on the Internet and see at which test you think you'll be able to perform the best.

If you did, then they'll just compare it with your diploma. Often they just write the correct title and this is not an issue.
Some of your questions have already been answered in my earlier posts, but to guide you along, here's my quick reply.
You can already start sending your application forms, including the education form, to CGFNS. Enclose the bank draft and copies of your diplomas.
If you know someone else who's also applying for Illinois, you can send your documents together in one Fedex envelope. Just have separate brown envelopes. This way you can share the cost of shipping.

you can just submit the ED-NUR for your CES application. in my case, CTS didn't require the ed-nur anymore since i had completed my CES already.
you can submit the documents that you already have and CGFNS will put these on file and assign you an ID number. But the formal review of your documents won't start unless you have completed everything, including the license validation form. so, the long wait is really only for your license validation form.

you're right, a local license is not required for registration in Illinois, BUT if you have taken it you have to declare to the Illinois board that you took the exams, whether you passed it or not. This is the "legal" thing to do. You don't want something like this bite you later when you have already processed everything. This is something you have to think about seriously if you're thinking of shortcuts.
Those who didn't take the boards actually finish their applications MUCH faster. They get their eligibility in about 2-3 months. But if anyone is thinking about hiding their license status, better think about possible consequences later on. There might be none or you could end up in a very deep hole that's hard to crawl out of. You have to weigh your choices everytime.

That's actually a feasible option. Pwede...pero.. it's a very expensive way to get a license if your end goal is to work in Illinois. If you endorse from another state to Illinois, you'll still need to secure a CES report, so you'll pay for your application in the first state and then pay for your licensure application in IL.
Actually the entire processing time to get a license in IL by going through endorsement instead of just waiting for your PRC license is almost the same. The only advantage I see is you'll probably take your NCLEX faster if your chosen state doesn't require a lot of documents.

you'll need to declare your first course. if i remember it correctly you should put it down in the non-professional education part of the application form.
i'm not very sure about license validation for non-nursing licenses. my guess is they won't require it anymore. however, please please email cts just to be sure. you can fidn the contact info in the resources page of my blog.
So, you can take either TOEFL iBT or IELTS. As long as you get the required scores, you won't have any problems.
Like I said, you'll need the PRC license card to complete the license validation form

we took the toefl ibt because we were considering going to grad school in the US. but if you ask me whether ibt is harder than ielts, i really wouldn't know based on first-hand experience because i haven't taken the ielts yet. from what i gather from other people, more people are comfortable taking the ielts. i think if you're not very confident about your english skills, you should follow the advice of others and take the ielts.
im planning to apply for the Certification Program(CP)of CGFNS and NCLEX at the same time. ive read that the CP includes the CES. do i need to take the CP first? coz it wud be very expensive if ill apply for CP($418) and CES ($378)which is needed for NCLEX at the same time, what should i do? thnx and more power to ur site. ;o)

for most states, well, at least 23 states, you don't need the CP anymore. but if you really want to take it, I know that the CP already includes credentials evaluation and you won't need a separate CES application. Just pay for the CP and that's it. For states that require only the CES, you can use the CP instead.

1. You can put CTS or Continental Testing Service as your receipient.
2.Check your diplomas, if it says Katunayan or Katibayan or if its in English, then write that down.
3. You're applying basically for licensing or you can put in employment in the purpose of your report
4. Part IV I think is the license verification. You need to wait til your local license is released (if you took the boards) and then have the PRC handle that form. Read some of the comments above for more info on this.
5. Child support does not apply to us, so just put N/A or No.

I just want to clarify something about the Part IV of the NCLEX application form (for CTS). It is the "Record of Licensure Information" part. I meant, instead of using "il.gov", you should have used "illinois.gov". so, the correct email address should have been "fpr.prfgroup09 (at)illinois.gov".
NOTE: I am reluctant to write the full email address with the @ sign because I don't want IDFPR's e-mail addressed spammed. Similarly, I advise everyone to avoid posting your e-mail address online as online bots collect these information for spam purposes.
The process is fairly fast. The longest wait is for the License Verification. The PRC doesn't issue verification unless you already have your license card and that can take a long time. But once you have all the requirements for the CES, it can be issued in less than a month. Your eligibility will also be issued in a few weeks.
Because there is a long wait for the license verification, what we did was to submit all the other documents that were needed for the CES and while waiting, we took the English test first. This way, no time is wasted, so to speak.

Another question, ilang days bago mag expire ang ATT? I just want to have an idea at least mai budget ko yung review days ko. Is it 1 year ba? I read from somewhere it varies from state to state, some its 1 year others 60 days and 90 days.

IL license application is pretty straightforward. there are more than one agency that you have to apply for that's why it gets a bit complicated. but whenever you encounter a problem, just post a message here and I'll try my best to answer them.
The ELIGIBILITY expires in 1 (one) year, you have to register with Pearson Vue within that time. But once, you have already registered and paid the $200 testing fee to Pearson Vue, they will issue you the ATT, which is valid up to 90 DAYS. You have to schedule an exam date before your 90 days is up.
When you claim your license card in PRC, bring the License Verification form from your CES application. Fill up the appropriate data. Photocopy your license and go to Window 1 or 2 (I think Windows 3 or 4 also) and show them the form. They will give you a Yellow Form. Fill that out and I think you have to pay for the stamp (I forgot what it's called.) Ask about Fedex shipping and they will either schedule pickup and delivery. Bring about $1,500 for Fedex services. If you have a friend who's also applying for IL, you can ask that your docs be sent together. This way, you'll be able to share the cost for mail services.
About dun sa fingerprint card, i got mine around october ata, di ko lang naasikaso kasi, other concerns came up kasi, ok lang ba kahit oct ko pa nakuha yung fingerprint form? Dun ba sa CES, is this the 2007 CES application handbook?

If you apply for express service, you can get it within the day. You can choose between the regular courier service or Fedex. Ask about the Fedex service. Usually, your document will be scheduled for pickup and you'll pay the Fedex rep directly.

For questions 1 to 4, you've done everything right.
For #5, just accomplish the Education review and License Validation for the CES application.
Technically, you can send the Education and License forms the same time. But you'll need to have your PRC Nurse ID ready before you can do license validation. Check out the posts above for more info on License Validation.

you can opt to accomplish just the license and education verification for the CES and disregard the same forms from the IDFPR application.
you may choose to send the license validation to IDFPR as well, but it's not really necessary because the same information will be reflected on your final CES report.

the school should send your docs, but if their policy is to let students do it, ask them to seal the docs in official school envelopes, so you can send it thru the nearest courier service. usually, from experience it takes the school a couple of days to months to prepare the TOR and fill out parts of the CES form, so I suggest to use charm and wit to convince to work faster. Har har har
Kidding aside, you can send your docs as they become available. You'll be sending the TOR, License Validation and application separately, and also the fingerprint card, so send whatever document you have ready.
No need to print in color or use special ink. Blank ink will do just fine.

It's better to finish everything before you leave. This way your eligibility will be ready when you reach the US.
There have been cases where someone on a tourist visa was sent home, perhaps there was a reason for the immigration officer to suspect that the person is intending to stay in the US for work and not just to visit. An eligibility to test, or even a passing NCLEX score, will not show up on the immigration officer's computer, but other things, for example declaring that you are a nurse or other physical evidences in your possession at the time of the interview, may be used however.
One more question.... Iam now reviewing in merge urc... Is it true that photocopied materials are also not accepted in fedex? Do I have to type all the review materials just to be safe... thanks a lot..
I would assume Fedex will any type of document. They normally don't go through every page of your parcel so it doesn't matter.
Place your reveiew materials inside an envelope if you wish.

the fingerprint card should be sent to IBT. please check with IBT directly how much their rate is now, because it might have changed.

The authorization is needed when you can't personally manage your own application. for example, i you have a lawyer or perhaps a relative whom you want to receive your documents or call CTS in your behalf.
Just leave that blank. :)

Your school records should be sent by your school. however, some schools don't have their own mailing service, so you must ask the registrar to put your documents in a sealed envelope and you should send it at the nearest postal office or courier service. asking a relative to send it from the US will only create suspicions that your documents may not be authentic.
with regard to your relative sending documents in your behalf: unless your relative is enclosing a check to pay for the services, I highly suggest that you skip it entirely and send your application directly to the appropriate agencies. sending it to someone else only creates delays and opens up other risks such lost mail or crumpled forms, among others.

Payments to CTS can be usually done using a bank draft or international money order, if an applicant is in the Philippines. Otherwise, a check, money order, or cash paid directly at the agency's office can be used if the person is in the US.
The TOR should be sent together with the education record forms.

I've always held the opinion that it is simply better to apply directly in the US state where you want to work in. This is my own opinion and I won't tell you to follow it.

if you wish to submit a copy of your TOR and the ED-NUR form,, it has to come from the school and sealed in an envelope if they can't send it on its own.
but you're right, you don't need to send the ed-nur form since your CES report will reflect the information CTS needs.

it usually takes CTS sooner than two months to process your eligibility. i'm sure there are updates to your application. there could just be some delays in the mail service. I would suggest calling them or e-mail them and ask the status of your application.
are you ready to take the nclex? since you've got most of the reqt's done, I would also suggest that you register with Pearson Vue already so that you can get the next step in the process started. Remember though, that once Pearson issues an ATT, you have three (3) months to schedule an exam.

i agree, that part gets a bit confusing. the application forms, the education record, the license record, and the fingerprint card should be sent separately. if your school can't mail your records, have the documents sealed and mail it yourself. the license record, however, must be sent through the PRC. once you get your license verification form, you have the option of either sending it through regular mail or Fedex right inside the PRC compound. A Fedex rep comes to the PRC everyday but ask the PRC staff for the next pickup schedule. You pay directly to the Fedex rep.
The latest I got from mary hazel, another IL applicant, is that IBT is charging $49.25 for the fingerprint scanning. you may confirm that with diana smith of IBT. her contact info is in the Resources page of this blog.
This is a common error which usually delays the release of your CES. No need to fret, this won't disqualify you. You will need to submit another set of application forms. Check which form (CES application form vs. Education Record form) has the wrong data and re-submit those with the correct information.
I'm not sure if you still need to send another form or if CGFNS would just coordinate with your school. Better to ask them directly by sending them email, which would be the best course of action at this time.

The confusion comes from a recent fee change implemented by CGFNS. Previously quoted fees did not include the additional $50 processing fee. Now they've added the additional fee in their literature.
did the price change for CTS? I mean, from $79 to $82? thankss :D
i just looked at the CTS Web site and looked at the application packet. Both says the application fee is now $82.
I just receive a confirmation letter from CTS and its also indicated that they havent received any docs from CGFNS yet. My CGFNS status is still "ready for review". Will there be any problem with that? I mean, ive submitted everything to CGFNS and CTS. Or shall I contact CTS about it?

that means you're almost set. there's no need to call just yet. your CES report should be ready soon and this will be forwarded to CTS once it is available.
focus on studying for the nclex instead, so that when your eligibility arrives, you'll be prepared.

*ask ko lang po...plano ko kc magrequest ng fingerprint card from US embassy then mag papafingerprint ako sa NBI carriedo. after ko macomplete ung fingerprinting i plan to send it directly sa CTS...possible ba itong option na to?
