// How To // UPDATED Applying for a nursing license in Illinois

UPDATE: In Sept. 2011, IDFPR made some changes to the application guidelines that affect internationally-educated nurses. Updated information is provided after each step, where necessary.

Here's a quick rundown of the things to do when applying for a nursing license in the state of Illinois if you're a foreign-educated nurse:

1. Apply for a Credentials Evaluation Service (CES) report from the Commission on Graduates of Foreign Nursing Schools (CGFNS). Request for a Course by Course evaluation or the Full Education report. Download the application form from the CGFNS Web site (http://www.cgfns.org).

*** You may apply for a CES with either CGFNS or with Education Records Evaluation Services.

2. If your school's medium of instruction is NOT English, apply and take the TOEFL iBT (www.ets.org/toefl). How do you know if you're school's medium of instruction is English? Aside from trying to recall if your teachers taught in English and made you read books in English, you can ask your registrar. The medium of instruction is required in the CES report, and this part will be filled in by your registrar or dean. (See instructions in the CES handbook in the application form.)

*** In the official IDFPR application guideline, it states that if your medium of instruction and textbooks are in English, the English language proficiency exam may be waived. However, I have not seen or heard of a case where the IDFPR has waived this requirement, so you may have to ask them personally as this will be reviewed on a case-to-case basis.
*** IDFPR now accepts IELTS exam scores, aside from TOEFL. In my opinion, you may want to go for IELTS as this test is recognized as well by other regulators outside the US, such as the UK, Australia, Canada and NZ, among others. TOEFL is only recognized mostly by US-based institutions.

3. Apply for state licensure by completing an application form (http://www.idfpr.com/dpr/apply/Rn.asp) and sending it to Continental Testing Services (CTS). Before sending your application form, request for a fingerprinting card from the Illinois Department of Financial and Professional Regulation.

To request the card, you can send an e-mail to Ms. West through her mail: FPR.PRFGROUP09(at)illinois.gov. Please ask kindly when you e-mail her.

*** It's been years since I last communicated with Ms. West and I am unsure if she still works for   IDFPR, so you may want to check this first. If you have new contact information, please send me a message so I may update this information.

When you receive your card, fill it up and go to NBI Carriedo. Buy a P20 ticket at the gate and then proceed to room 604. You will be given a piece of paper which you'll show to the fingerprinting person on the 5th floor. When you're done, send the fingerprinting card to Integrated Biometric Technology:

*** The NBI may have changed their fees and their step-by-step workflow. Again, if you have newer information, do share with us here.

Check IBT's Web site to find out how much they charge for the fingerprinting processing. Enclose a note to request IBT to send the receipt to CTS.

Each step requires you to pay certain fees. The list of fees is printed on the application packets. If you're in the Philippines, you can pay through bank draft. Go to a bank where you have an account and ask how you can get a bank draft. Before you go to your bank, make sure you know the exact name and address of the institution to which you'll be sending the payment and the amount, of course. Once you have the bank draft, send it together with your application forms to the respective organizations (CGFNS, CTS, etc.)

Common questions:
1. Do you need to take the CGFNS certificate exam?
No. You don't need the CGFNS exam. However, if you already have one, you can ask CGFNS to submit that to CTS in lieu of the CES.

2. Can you apply for the CES and the state licensure with CTS at the same time?
Yes. You can submit the forms at the same time, but your application won't be processed unless the initial steps are completed (CES then TOEFL then CTS application.)

3. Can you take the IELTS instead of TOEFL?
UPDATED Yes. The Illinois nursing board now accepts TOEFL AND IELTS scores. As mentioned earlier, if your CES report indicates that your medium of instruction and textbooks in school were in English, you may have the English test waived, HOWEVER, you still will need a passing English test score for the Visascreen application later on.

4. Fedex doesn't deliver to P.O. Box addresses. What address can I use to send my documents to CTS?

Some have successfuly requested fingerprint cards from the local US Embassy. Although, I don't know anyone personally who has done that, I've heard many say that the cards are accepted abroad.

The fingerprint cards are not sent to CTS, instead you need a service provider to digitally scan the cards. The information you need is already provided for you in the how-to guide above and additional information can also be found in the comments space.

ei geoff, i've been e-mailing cts (ms. susan zajda) to inquire regarding my application to take the nclex-rn many times over and yet i don't get a reply. it's been more than two months now and still i don't have my eligibility. i'm really getting impatient coz i'm through with my review since a month ago. i need to take the nclex-rn the soonest time possible...

Ask ko lang ulit, magkano po ba ang bayad sa L1 identity solutions para sa fingerprinting card services. wala kasing corresponding fee para sa mga services nila doon sa website nila. Ano pa ba ang mga accredited IBT's sa Illinois? pede po painclude kung anung site ko makikita ung corresponding fee para sa services ng L1 identity solutions... salamat sir Geoff! ^_^

The IDFPR usually answers e-mail within a few days. Don't expect an immediate reply as some of these agencies may have loads of work too. If you don't get a response in a week, look for other options for the fingerprint card. Some have tried the US Embassy in Manila. Although I don't have personal experience requesting a crd from there, some feedback I get have been positive.

ei, geoff my sis called cts... ms. zajda told her that there's nothing wrong with my application requirements. the reason why they're not issuing my eligibility is that i'm not yet registered with pearson vue & not yet paid the$200 fee... i thought it would be better to wait for the eligibilty first before i register with pearson vue... well, at least i know now what to do... tnx for the reply to all my inquiries! i'm just glad i chanced upon your blog especially at a time when my life is at it's abyssmal low... tnx again!

The English exam is needed when you apply for a Visascreen, which means you don't necessarily need to take it before your NCLEX. You are exempt from the TOEFL requirement of the IL BON because the medium of instruction in Phil. schools is English.

Unless you really want to take the TOEFL, you may also take the IELTS, which, according to other people, is relatively easier than the TOEFL iBT.

Hi Geoff,

I already took my nclex-rn and ielts exams and passed. So, I am now about to apply for a visa screen maybe next month (feb). My question is, how long will it take for the CGFNS to process it? Co'z I have a US-tourist visa, and I'm planning to just go to US and have it process directly there myself instead (if it takes more than 1 month to have it processed and just wait here in the Phils.). What can you advise?

The processing time for the Visascreen depends a lot on the time CGFNS receives your documents. Were you applying for a license in Illinois? If so, then you have your CES report, right? If you have one, then you'll just have to wait for IDFPR to issue you a license and then a certification.

If you can provide more information about the documents that you already have, then I might be able to help you better.

As for coming to the U.S., you must be aware that no visas are yet available. So, unless you can get a change of status within your legal stay in the U.S., I highly advise against overstaying there. Once you are out of status, your immigrant or work petition will be automatically denied once it is filed.

thanks for all your help and support and i was able to send all my documents :) i've received an email from CGFNS, though its only an address/phone confirmation email.

im just wondering, should i receive any postal mails or emails soon?

you can register with Pearson Vue when your CES report is ready or almost ready to be issued. CGFNS usually sends out snail mail but don't expect that to be quick. You know how the post office works :)

I'm glad you found the articles helpful. It doesn't really matter how many schools you attended before you got your nursing degree, just make sure that your last nursing school consolidates all your grades from your previous schools in your TOR. In your application, just write down all your previous schools and degrees.

you can submit your application from Saudi, however you will have to retun eventually to the Philippines because your immigrant visa will be issued there, unlike when getting a tourist visa where you can apply in the country you're at. with the immigrant visa, you have to go back to your country of citizenship to be issued a visa.

1.what if i haven't received my cgfns id number but i already passed some of the requirements (high school diploma,college transcript, PRC from) for CES. I passed it last december and up to now, i haven't got any confirmation e-mail. do i have to wait for my cgfns id number first?

I'm not sure I got your question. What do you want to do with the CGFNS ID number? It is usually sent by postal mail, but you can go ahead and create an online account in the CGFNS Web site. Once you've created an account, your ID number will appear on the Profile page once the agency updates it.

I think there's a limit to the number of fingerprint cards you can request, but I'm not sure how many. If you get all five, then that's good.

When you graduated from Grade 10, is that the equivalent of a high school graduation? you can simply put the date when you finished grade 6 as your grade school graduation and grade 10 as your high school graduation. As for your diploma, I suggest send the affidavit as well. Usually, we're exempt from the HS diploma requirement, so I believe it won't matter so much.

Have you completed all the documents needed for the CES report? If you created an online account with CGFNS, you should see if your documents are ready for review. Once they are, it shouldn't take more than a week or two to finish.

I got my CES report already. In my primary education it says there 6 years primary education. But i have 7 years of primary education since we have grade 7. Will there be any problem with that? Shall i report it to cgfns?

Once your CES is ready and you've complied with other reqt's of CTS, your Eligibility usually follows right away. To complete the process, CTS wants applicants to register with Pearson Vue. If you feel you're not yet ready to take the NCLEX, then just prepare first before making that Pearson Vue registration. After all, once you register you only have three months to sit for the NCLEX, otherwise your registration will expire and have to register and pay again.

hi geoff nico here....

i have checked cgfns.org and it is posted there that my REPORT has already been issued. I have also sent my CTS application to CTS and finger print cards to ibt.

The problem is i haved received a confirmation letter from cts that says that they have received the application form but still lacks the page 3 which is the license verification and the CES report. I have sent the CTS application a month before the CES has been issued.

I havent also received any confirmation from ibt for the finger print cards... should i receive any?

what should i do next should i register now to pearsonvue? What should be my next step?

Iam now done with the ielts and review for nclex that's why im really excited to take the nclex as soon as possible...

You can now register with Pearson Vue online. You'll get your ATT in your e-mail. Once you have that, you can call up Pearson's customer service to schedule an exam date.

The CTS letter was obviously sent before the agency received your full documents.

Register with Pearson Vue so you can finish the application process. IBT only issues a certification to CTS that you've submitted your fingerprints to them. This is usually a seamless process.

Your CES report has been issued and that means you are almost ready to become eligible to sit for the NCLEX exam. The last requirement is for you to register with Pearson Vue and schedule an exam date.

When endorsing a license to Illinois, I believe you still need to get a CES report and accomplish an endorsement appplication form. Download the form from the idfpr.gov Web site to see the complete instructions as well as the fees.

You can ask the PRC to send your verification through Fedex. Usually you meet up with the Fedex rep and pay directly to them once your license verification has been released. Nov is such a long time, unless your license is not yet ready. Did you have a license card already when you applied for the verification?

There are some additional info in the Comments area, I hope you can scan it also.

The info you need is in the CTS/IDFPR application packet, if you wish to confirm the English requirement waiver.

You specify which agencies receive the CES report when you applied for it, so if you specified CTS, then they will receive a copy of the report also. I suggest registering with Pearson Vue when the CES report is ready to complete the eligibility application.

I have been trying to call suzzane of cts but only the answering machine answers me and i havent received any reply fro cts..

thank you so much for the info on how to apply for d nclex in illinois! ur site really help me on processing my application. and it only took me 2 months to have my ATT at hand once i submitted all d requirements that u told me.. i dont think that i will able to go through this without ur help. as a result, i will be taking my nclex exam after 2 months. i just want to ask for your advice b'coz, i have already bought a book on saunders and a cd (4th edition) and my brother has also send me a kaplan book/cd 08-09. are these enough for me not to go to a review center? and if you prefer me to apply n a review class, which Review centers, u think, would be most effective? and i'll be starting to work also on a hospital next week at UDMC near rotonda (manla-qc boundary) which i believed that merge and sti conducts review classes for nclex.. which you think s d best?( i heard that merge has no simulation examination on computers, they only conductlectures while STI do have a simulation exam on PCs but no lectures) thanx again geof!! you really are a great help! more power at ur site!!

anyway, here's my dilemma.. i juz started doing all the paperworks for my nclex exams.. i just send my ces to my school's registrar and is currently filling up the forms so i can directly send it..


let me just clarify some details...

1. am i ryt in thinking that in general, what i need is a fingerprint card processed, a english exam (TOEFL,which i already passed)and the ces.. and i just need to send this all to the state board.. The fingerprint card, i'll send to IT (im applying for illinois btw).. the ces, i'll send to CGFNS????


2. about the TOEFL, i passed it last nov. this may sound stupid, but am i supposed to receive anything with regards to my passing the exam? we moved kasi ryt after taking the exam.

3. with regards to the packet, it includes the the ED Nur, the CT NUR and APPLICATION FOR LICENSURE OR EXAMINATION.. the CT nur is for the PRC, the ed nur for my school and all of these PLUS te application for licensure papers to the continental testing.. ryt????

4. about the part 4 and 5, what do i write ther? i just need to leave it blank ryt? the instructions in the packet ksi is soooooo confusing.

5. also, would you mind explaining the att? who gives it? and how do i get it?? (if you can, can you explain the whole process to me step by step??? only if its not a pain??=))

6. if i do have the ces na, filled by my school, I DONT NEED TO PASS THE ED NUR NA ryt? or can i also send it?

7. and lastly.. with all the papers ready for sending, in your calculations, how long will it be before i schedule my nclex exams?

This is the first I have seen of your blog and I am very impressed. Especially at your explanation of the Illinois process. As the CEO of a U.S. recruiting and placement company with offices in the Philippines, it is nice to see accurate information for a change. My company does have contracts ini Illinois and I just want to reiterate what you said about the IELTS. As long as your nursing curriculum was administered in English the IELTS is perfectly fine in Illinois. Fortunately for the majority or Pinoy RN's, your education was in English. This was a major concern of mine when I first started doing business in Illinois because the IELTS is the only English exam I use. I have had no problem with our nurses getting their licenses at all.

NCLEX results for IL applicants can be viewed 48 hours after the test through Pearson Vue's quick count. See their Web site.

The CGFNS Cert. exam is no longer mandatory for IL licensure. You can get a CES Report instead. Look into the CES report from the CGFNS.org Web site.

You do not need to have a Phil license to become eligible for the NCLEX if you wish to get a license in IL. The CES, which is one part of the eligibility process, does not require a local license.

Just follow the steps we've prepared here and read the comments to help you figure things out. Goodluck.

just some questions geoff, i followed you advices and complete na ngaun lahat ng papers ko na kailangan isend like the CES report, Fingerprint card ( nagrequest ako thru mail kaya aun after 2 weeks pinadala na nila ), and ung application form para sa NCLEX exam and the VISA screen application form ( kaso later ko pa ata to kailangan, am i right? )..

and my questions are?..
1. May part dun sa booklet na kailangan ng request for validation of registration... ung para sa PRC, gusto ko na sana magsimula kaso wala pa akong license hndi pa lumalabas ung result ng boards for june 2008.. panu kaya tong part na toh?..

2. tapos sa part 13 o no. 13 dun sa booklet na hawak ko inaask nya:
a. if you are not currently registered /licensed, please indicate and explain.

4. anu po ung ilalagay ko sa nursing school program code tapos nasa baba nya ay NCSBN Number ang hinihingi?..

5.and i know from the older posts naka lagay na dun na i wont be needing english exams like toefel or IELTS, kasi our schools medium of instruction is english.. panu po ung sa visa screen ileleave ko na lang ba na blank yung mga un?..

Since you took the local board exams, you have to declare that you took it, so you have to go through the License Validation process.
1. Wait for the results of the boards and I believe you can get validation even without the license card itself. You just need to show the paper slip or certification that they give out at PRC when you pass.
2. So, number 2 no longer applies to you.
3. You have to accomplish the license verification for the CES, but if I remember correctly, you can skip the verification with the CTS. Try to read up past comments and posts here.
4. Leave the code blank if you can't find it.
5. You still need to pass the English test for the Visascreen, but not to get NCLEX eligibility in IL.

If you already have a CGFNS certificate, then you don't need to apply for CES. You can use that to apply for registration with CTS.

The Visascreen requires a passing score in an English exam, so that is a requirement whether applying for the first time or renewing an expired VS.

To retake the NCLEX, you don't need to re-apply for a CES report. Just register again with IDFPR/CTS and they'll tell you what to do.

hi. goodpm. your blog has helped me in appying at CTS. quick question tho, since i put my Phil SSS #, can it be the reason why i cannot view my status online? is it possible that my information wont be in their database at continentaltesting.net? because i mailed my application together with my livescan and money order last july 10, (btw im in chicago, on a tourist visa) they encashed the money order last july 16 and yet i cant get my status. my ces was long overdue, i think they mustve got it in april. you think they might have lost it, because i just recently filed my application? thanks!

A US SS No. was what was being asked in the application. If you didn't have one, you should have left that blank. Anyways, that is not the reason you can't see your status online. The agencies usually don't update their online databases as fast as we wish they would. Just send them an email or call them since you're in Chicago already.

Paying online with a trusted site is actually safer than handing a credit card to someone you don't know inside a restraurant or store, so feel safe when paying online with Pearson Vue.

The fee is $200, plus $150 if you're taking the test outside the US. You pay the $200 first when you register, and then pay $150 when you schedule a test date. When scheduling on the phone, have your credit card or your sister's card with you.

hello.. i'm applying for IL and i have some problems with the fingerprint card. A friend of mine sent me a card. She got it from IL state of Police. Is that okay? Do I need a verification that my fingerprints were taken from the NBI? Another friend who applied in IL told me that there is a space where an NBI officer needs to sign. I'm confused. Do the fingerprint card forms differ?

I just have a question, my cousin is in IL, she told me to take the NCLEX of Vermont instead of IL, because SS number is needed in IL in able to renew the license. Is it true? Since there is ongoing retrogression, she worries that it might expire and I am not able to renew it since I don't have any visa. She told me to apply it for a reciprocity if I will be able to pass the Vermont's Exam and if US is already open for nurses again. Is it a good way? Is it easy to apply for a reciprocity from Vermont to IL?

Also, CGFNS is not a requirement in IL? But what is CES for, that I should get in CGFNS Org?

Is it okay to take up IELTS instead of TOEFL?

The 9999 is merely a dummy number that the licensing agency uses. You should have put NONE because you don't have a SSN.

I am planning to apply for the NCLEX-RN in IL. I chose ERES for my CES report and mailed the school forms (FORM101) and my application form (FORM100). I'll be going to PRC ILOILO (since I'm Bacolod-based) tomorrow for the FORM102... wanted to ask you:

1. How long will PRC ILOILO send the form102 directly to ERES?

2. I used the local postal service instead of FEDEX, etc to send my forms. They say it takes 15 days to get to the destination, is it true? Airmail naman....

3. When sending the 4 page Application form (IDFPR) to CTS, how can I pay? Bankdraft with the form? To whom do I address the bankdraft? And I'm confused, is it $82 or $200 or $282?

4. If I pay that $82 with the forms, then I won't have to register online at CTS (www.continentaltesting.net)?

5. If I pay the $200, would I still need to pay $200 for the PEARSON VUE when creating an account?


6. I also need to confirm that I DON'T have to send the CT-NUR and ED-NUR anymore since I have CES (ERES).

You've just been issued the CES report, which basically, is just a report that determines if your education is comparable to US standards.

CGFNS should have already sent a copy of your CES report to CTS, which will in turn determine if you meet all the requirements for NCLEX eligibility. , for example, fingerprinting, is one of the other requirements, among others. Without the CES report, CTS cannot evaluate your application fully. All the required documents, fees, etc. must be met first.

It usually takes CTS a few weeks to complete its evaluation. If you know that you've completed all these requirements, I suggest calling CTS two to three weeks after the issuance of your CES report, just to follow up on your application.

Once you receive your eligibility, you can register with Pearson Vue, and then wait again for your ATT. Some applicants get it in a matter of hours, for some, it takes days. After you get your ATT, you can call Pearson Vue's customer service to schedule an exam date. I suggest securing that US Visa first before you schedule an exam in the US. Unless you already have a US visa on hand, you could risk scheduling an exam date in the US and later on being denied your visa application. You can start applying for that US Visa now but have another valid reason to go to the US aside from taking an exam which you can sit for without leaving the Phils.

ERES is one of the agencies accredited by CTS that offer the CES report. You may use their service if you wish. One advantage to using CGFNS is that you can use your CES for other states that may require the CES report. However, if you don't see that happening, you may consider ERES and what they offer.

I've noticed that the addresses on the forms I've sent (both CTS, ERES, etc) have no U.S.A. on them. Just a copy/paste address from their forms which are only the State and zip codes. I forgot to place the "U.S.A." on them, I am using the Philippine Post Office, would it reach the destinations correctly?

I have already mailed Ms. West re:my request for the fpcard almost a month ago, she did reply back anyways. but still i'm worried coz it's taking so long, is it usually this long for the fpcard to get to me? ayoko lang mag-antay ng wala eh.

But when i gave the CT-NUR form and instructed them na ibbalik sa akin ung form na iyon for it to be attached sa APPLICATION FORM ko hindi daw pwede dahil nga nakasulat sa CT-NUR form na "DO NOT RETURN COMPLETED FORM TO APPLICANT". i even showed them ung instruction sheet and pinabasa ko pa ung hinighlight ko na statement dun sa page3 ng instruction sheet "You must direct the appropriate licensing agency(s)/board(s) to return the
completed form directly to you to be submitted with your application."

should i let them send na lang ba the CT-NUR form sa CTS?

im sov, a newly licensed nurse here in manila, i was told by my friend abt your site coz you really helpd him a lot during his application for nclex, and now he's an nclex passer for the state of illinois, my question is doyou also have a step-by-step application for nclex for the state of nevada? thats whre my sister is right now, so im planning to be a nurse in that state, althou i read the application for the state of ilinois, does the state of nevada difer?or can you link me to other site that may help me in my situation,, thank you and God bless!

i'm confuse kung anung ilalagay ko sa Name of diploma or certificate in its original language... wala pong nakalagay na katibayan or katunayan sa diploma ko... i don't know what to write... =(

I have a questions,hope u can help me.Im studying nursing in the philippiness right now and ill be done in 2013.Im Fil-American and been in the states for 10yrs.My question is: do i have to take the licensed in the philippines before i can apply to take nclex in illinois? Can i just go to CGFNS to get CES and apply for the nclex kahit wala na or no philippine liscense from PRC because ill be working here in the states...w/c is my plan here in illinois.Thank you and hope u can help me.

Hi Brenda,
There was a change in IDFPR's policy recently. I suggest you contact them directly before you do anything. From what I understand, they will require that you are licensed from your country of education. I don't have the new guidelines right now but I will check when I get the chance. But the best I can recommend is that you contact them directly. The phone numbers are posted here in my blog

i passed NCLEX septmeber of last year. i submitted the reply slip and payment to the dept of regulations and yet i'm still not registered in their website and i haven't heard from them eversince:( i was wondering if i need to take IELTS? I didn't plan of getting it coz i thought it was just needed for the visascreen; I'm being petitioned by my fiance. is there a new rule in IDFPR that before i get my license i should take IELTS? or is there any documents that i need to submit? i tried calling them but really, it's no use. no one's answering and i'm left hanging. please help me, i don't want my NCLEX exam to expire. thanks so much!

The process of getting licensed in Illinois is pretty lengthy, but I believe it's still the same. They do require that you have a current license in your home country, and I can't remember now if it wasn't required in the past. They do emphasize that now though. Some things might have changed like accredited providers, etc. Make sure you download an application packet first from IDFPR to know where to start.
